Definition
The Fraud and Abuse Control Information System aggregates state and federal healthcare-licensure and exclusion sources. Level 1M covers federal exclusions (OIG-LEIE, SAM.gov, OFAC); Level 2M adds state Medicaid exclusions and licensure boards; Level 3 is the most comprehensive and is the standard for direct-care providers.
