Personal Background Check Agreement

1. Parties

This Personal Background Check Agreement ("Agreement") is entered into by and between SafestHires and the individual identified in the Agreement Confirmation below ("you" or "Consumer") who is ordering a background check on themselves.

2. Services and Price

SafestHires agrees to prepare a personal consumer report on you based on the information you provide. The flat price for this report is $44.95 and includes:

• SSN Address Trace
• Nationwide Criminal Database Search with 50-State Sex Offender Registry
• County Criminal Search in up to three (3) counties of your choosing

SafestHires will deliver the report to the email address you provide. The report is for your personal use only and is not a substitute for an employer-ordered background check governed by the Fair Credit Reporting Act (FCRA).

3. Your Responsibilities

By submitting this Agreement, you confirm that:

• You are at least 18 years of age and are ordering this report on yourself
• All information you provide (legal name, addresses, counties) is accurate and complete
• You will use the report solely for your own personal review and will not represent it as an employer-ordered FCRA consumer report
• You authorize SafestHires to query public-record, criminal-history, and identity-trace data sources to compile your report
• You will pay the $44.95 fee at the time of order

4. Accuracy and Limitations

SafestHires sources information from public records and third-party databases. While we take reasonable steps to ensure accuracy, records may be incomplete, delayed, or contain errors at the source. The Nationwide Criminal Database is a pointer database and is not a substitute for a primary-source court search. County criminal searches reflect records held by each county court at the time of search.

5. Disclaimer of Liability

SafestHires shall not be liable for any claims, damages, or losses arising from the use of information provided in your personal report, except in cases of gross negligence or willful misconduct. You assume all risk and liability associated with how you use the report.

6. Refunds

Because reports are compiled and delivered shortly after order, the $44.95 fee is non-refundable once the report has been generated. If SafestHires is unable to deliver a report for technical reasons, you will receive a full refund.

7. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Colorado, without regard to its conflict of law provisions.
